You don't need ten thousand steps - that number came from a 1960s pedometer's brand name, not from science. The evidence points to a much lower sweet spot, and most of the benefit has levelled off by the time you reach it. You also don't have to hit any target to gain: the relationship is a smooth gradient, so every extra thousand steps a day counts. The fix is to add one short walk to wherever you are now.

The comparison here is with 2,000 steps a day, not with nothing and not with an average - so this is the gap between someone barely moving and someone walking a moderate amount, which is a wide gap and part of why the figure is large. The shape of the curve is often described more simply than the review actually found it. For death from any cause, and for heart disease, dementia and falls, the curve bends and begins to flatten somewhere around five to seven thousand steps. But for five other outcomes the review examined - death from heart disease, cancer diagnosis, death from cancer, type 2 diabetes and depressive symptoms - the association kept going in a straight line, with no flattening found. So there is no single step count at which the benefit stops, and anyone who tells you there is has picked one outcome. What can be said is that ten thousand was never an evidence-derived threshold. All of this is observational: people who walk more differ from people who walk less, and illness reduces walking long before it is diagnosed.

How to actually do it
Check your current daily average - most phones already count it - then add about a thousand steps, one short walk of roughly ten minutes. Anchor it to something you already do: a walk after dinner, parking further away, a lap while a call is on. Once that's routine, add another thousand. The target isn't 7,000 today; it's up from wherever you are now.

Walking is usually argued for on the heart, and this is the part of the same review that gets quoted least. Two cautions belong with it. The dementia estimate rests on only two studies - the studies happened to agree with one another almost perfectly, which makes the range look reassuringly narrow, but two is a very small number of studies to rest anything on. And the direction is especially hard to read here: the earliest years of dementia change behaviour, including how much somebody walks, long before anything is diagnosed, so some of this association is very likely the disease reducing the walking rather than the walking holding off the disease. The depressive-symptoms figure comes from three studies and is the one outcome here where the review found a straight-line association rather than a curve that flattens.

Where "10,000 steps" actually came from
Ten thousand steps was never an evidence-based target. It came from the name of a Japanese pedometer sold around the 1964 Tokyo Olympics - "manpo-kei," literally "10,000-step meter." The number stuck as a marketing slogan and slowly got mistaken for a medical guideline. When researchers finally measured the dose-response properly, the health benefits didn't need anything close to 10,000.
What the step-count evidence shows
A 2025 meta-analysis found that about 7,000 steps a day, compared with 2,000, was associated with roughly 47% lower risk of dying from any cause. Beyond about 7,000 the curve flattens - you keep gaining a little, but the big returns are already banked. So 7,000 is a better real-world target than 10,000, and far less discouraging if you are starting low.
Every thousand steps counts
You don't have to reach a magic number to benefit. Pooling more than 226,000 people, each additional 1,000 steps a day was linked to about 15% lower all-cause mortality. The relationship is a smooth gradient from low counts upward, which is why the fix is "add a thousand to where you are," not "hit 7,000 or it doesn't count."
Why steps track with living longer
Step count is a simple readout of how much you move over a whole day, capturing the incidental activity that structured workouts miss. More daily movement is linked to better blood pressure, blood sugar, weight and cardiovascular fitness - the mechanisms behind the mortality association. Because the studies are observational, part of the effect reflects that healthier people walk more (reverse causation), so treat steps as a strong, trackable habit rather than a guaranteed dose.
What else the step count tracks with
Early death is the outcome that gets reported, but the review behind this page looked at nine. Comparing seven thousand steps a day with two thousand, it found lower rates of heart disease, of type 2 diabetes, of dementia, of depressive symptoms and of falls - and, for cancer diagnosis, it found nothing statistically significant at all, which is reported here because leaving it out would make the picture tidier than it is. The certainty the reviewers themselves attached to these varies: moderate for most, low for a few, and very low for falls. None of it is proof that walking produced these differences.

Is this proof that walking more makes you live longer?
No. Every study behind these figures watched people rather than assigning them to walk, and the biggest problem with that is obvious once you see it: illness reduces how much people walk, often years before it is diagnosed. So some of the gap between high and low step counts is people who were already becoming unwell walking less, rather than walking less making people unwell. The reviewers say so themselves, listing residual confounding among their limitations. What the evidence supports is a consistent association across many cohorts, in a direction that fits everything else we know about physical activity - not a demonstration of cause.
So is there a number I should be aiming for?
There is not, and that is the honest answer rather than an evasive one. The review found different shapes for different outcomes: for some the curve begins to flatten somewhere around five to seven thousand steps, and for others it carried on in a straight line with no flattening at all. Its authors put it as seven thousand being a realistic and achievable target for some people, while ten thousand remains perfectly reasonable for anyone already more active. What the evidence does not support is a single threshold below which nothing counts. Adding a thousand to wherever you are is the version of this that needs no target.
